Dedication Gifts

Donate in Honour
Special occasion gifts are a unique way to acknowledge a significant life event such as a birthday, anniversary, wedding, retirement or other milestone, while also contributing to a worthwhile cause. A lovely eCard with a personal message can be sent to the honoree notifying him or her of this special gift (donation amount is not included).

Donate in Memory
A donation in memory of a friend, colleague or loved one is a meaningful way to express your sympathy while also helping OCPSA enhance its programs and services. More and more people are requesting that their loved one be commemorated in this way in lieu of sending flowers. A lovely eCard with a personal message can be sent to the next of kin notifying him or her of this special gift (donation amount is not included).

Dedication Gifts can be made in the following ways:

Please click here to make your Dedication Gift online. Dedication Gifts can be made by a one-time donation or a monthly donation. Once you select the appropriate donation page, you will have the option to make your donation a “Dedication Gift”. Please follow the on-screen instructions from there.

Terms and Conditions

In consideration for the acceptance of my, my child/ward’s, membership in the OCPSA, I, the participant and/or parent/guardian (if participant under 18 years of age) agrees as follows:

  1. To adhere to all of OCPSA’s procedures, policies, and code of conduct;
  2. To adhere to OCPSA’s Concussion Management and Return to Play Policy;
  3. To accept sole responsibility for my or my child/ward’s personal possessions and athletic equipment;
  4. To accept all liability for any damage to the playing equipment caused by me or my child/ward’s careless, negligent and/or improper handling; and

To authorize OCPSA the right to the collection and free use of my or my child/ward’s images and athletic results for posting on the OCPSA website.
