OCPSA is pleased to announce a final list of athletes and alternates who have been nominated for Ontario Cards under the 2017-2018 Quest for Gold – Ontario Athlete Assistance Program (OAAP). The athletes nominated were selected using the sport specific selection criteria developed by OCPSA, approved by its Board of Directors and applied by (the) Selection Committee.

In consideration for the acceptance of my, my child/ward’s, membership in the OCPSA, I, the participant and/or parent/guardian (if participant under 18 years of age) agrees as follows:

To adhere to all of OCPSA’s procedures, policies, and code of conduct;
To adhere to OCPSA’s Concussion Management and Return to Play Policy;
To accept sole responsibility for my or my child/ward’s personal possessions and athletic equipment;
To accept all liability for any damage to the playing equipment caused by me or my child/ward’s careless, negligent and/or improper handling; and

To authorize OCPSA the right to the collection and free use of my or my child/ward’s images and athletic results for posting on the OCPSA website.
